How to submit a report 2025. Detailed instructions for lawyers: written and electronic forms

The NV experts of the Legal Goncharenko Center provided instructions on the requirements for submitting a report, the intricacies of the procedure, and document templates for various situations.

How and to whom should report

The report can be submitted both to the immediate superior and to the higher command. They write it in two copies and submit:

• to the formation unit of the military unit, (the incoming document number should be recorded);
• or to management that may receive it (record the date of submission, position and name of the person who accepted it on your copy);
• or sent by registered mail with a delivery notice and a description of the attachment to the address of the military unit.
• The report is submitted sequentially along the management vertical (for example, first to the company commander, then to the battalion commander, and so on).

“If the immediate supervisor does not respond to the report, the serviceman must contact the higher-level management, with a mandatory indication that he is trying to resolve this issue,” says Mykola Bryukhovetskyi, director of the Legal Honcharenko Center.

• Make a report.
• Submit the report for signature to the immediate superior in person.
• The commander signs the report.
• The report signed by the commander must be registered in the formation group of personnel.
• After registering the signed report, it is important to take the entry number of your report from the records of the military unit.
• The report is then submitted to the commander of the military unit for consideration.
• The commander of the military unit examines the report.
• Based on the results of the review, the report is sent for review or left without implementation.
• Next, the report is approved by the military unit.
• The report is sent for execution.
• The serviceman is informed about the results of the report.

How to submit a report online and Army+

You can do this through the Army+ application.

Step-by-step instructions from lawyers

• Make a report.
• Download the Army+ application and register on the website using this link.
• Find out the ID of your commander (for commanders — to find out SEDO ID in the training room).
• Select the necessary report from the list and send it to the immediate commander.
• Next, the immediate commander receives the report in the application, approves it, after which the report enters the SEDO of the military unit.
• After that, the report is submitted to the commander of the military unit.
• The commander of the military unit examines the report.
• According to the results of the review, the report is sent for review or left without implementation.
• Verification of the report by the military unit.
• Forwarding the report to execution.
• Notification of the serviceman about the performance results.

Currently, through the Armiya+ application, it is possible to submit reports for receiving referrals to VLK, MSEK, treatment, vacation and financial assistance payments.

How long the report can be considered

Reports are considered immediately, but no later than 15 days from the date of receipt, if it does not require additional study. The general term of consideration is one place, which can be extended up to 45 days. There is also an option to submit an application for expedited processing.

What should be the response to the report

The response to the report must be in writing. In the event that it is a refusal, with reference to legislative acts indicating the reasons for the refusal and explaining the procedure for appealing the decision.

It should be remembered that a serviceman or members of his family can also verbally address the commander of a military unit, the head of a military administration body, since they receive servicemen on certain days and hours for personal matters.
