Coming to the end of the first-half, Maddison chased down a long ball from Cristian Romero.
The midfielder got to the ball first and flicked a header goalwards.
But he was then wiped out by keeper Santos, who came charging out of his goal at pace.
His body clattered into Maddison’s head and he immediately collapsed to the floor in severe pain.
There was concern from his Spurs team-mates as they urgently called for medical assistance.
The horror collision was checked by VAR and a penalty was awarded for the foul and Santos was booked.
Dominic Solanke stepped up and coolly put Spurs 1-0 up, 2-1 on aggregate.
Maddison bravely tried to continue but had to be subbed off for Dejan Kulusevski.
Fans have praised him for bravely not ducking out the challenge, with one saying: “Massive shoutout to Maddison btw. That was incredibly brave because he looked and knew he was gonna be hit.”
Another added: “Maddison potentially could have gotten a brain haemorrhage from that challenge.”
A third wrote: “Maddison sacrificed himself for us to get a pen I won’t forget this bud.”
More commented: “Maddison, you WARRIOR. You risked your season to win that penalty.”
